Finding yourself in the hospital, a rehabilitation center or an assisted living facility may cause stress and worry. Often in these situations, decisions need to be made and you may need assistance during this time or in the future.

What You Will Need for Your Signing
Current Government-Issued Photo ID:
All signers must present a valid photo identification to ensure the integrity of the notarization process.
Your Document:
You should have a copy of the document you wish to notarize. Typically, the facility's social services office can assist you.
Sound Mind:
Signers must be of sound mind to execute documents legally. Also, a notarization should NOT take place right after (within 24 hours) a major procedure where general anesthesia was administered.
Credible Witness:
If a government-issued photo ID is unavailable, a credible witness may be used to verify the signer’s identity.
